Levonorgestrel in contraceptives and multipurpose prevention technologies: does this progestin increase HIV risk or interact with antiretrovirals?
نویسندگان
چکیده
Mounting observational evidence suggests that use of certain types of hormonal contraception, specifically the progestin-only injectable depot medroxyprogesterone acetate (DMPA), may be associated with an increased risk of HIV acquisition in women [1]. This relationship has been examined in several observational studies and is currently being assessed in a randomized trial, the evidence for contraceptive options and HIV outcomes (ECHO) study (NCT02550067). As evidence continues to evolve, it is also necessary to critically examine knowledge gaps for other contraceptive progestins. Levonorgestrel is used in many existing contraceptives and is being evaluated for use in multipurpose prevention technologies (MPTs). MPTs are designed to simultaneously prevent two or more of the following: unintended pregnancy, HIV, and other sexually transmitted infections. Potential drug–drug interactions and side-effect profiles must also be considered in developing products, including some MPTs, which would contain levonorgestrel and antiretrovirals. Risks of venous thromboembolism with various hormonal contraceptives.
Evidence There are no long-term RCTs, which leaves cohort and case-control studies as evidence. • The Society of Obstetricians and Gynaecologists of Canada1 and others2 have stated that evidence about the risk of VTE with different progestins is inconclusive owing to mixed results and a high risk of bias. • Some studies found no increased VTE risk with drospirenone3 or a transdermal patch.4 • S...
متن کاملContraception for the HIV-Positive Woman: A Review of Interactions between Hormonal Contraception and Antiretroviral Therapy
BACKGROUND Preventing unintended pregnancy in HIV-positive women can significantly reduce maternal-to-child HIV transmission as well as improve the woman's overall health. Hormonal contraceptives are safe and effective means to avoid unintended pregnancy, but there is concern that coadministration of antiretroviral drugs may alter contraceptive efficacy. MATERIALS AND METHODS We performed a l...
متن کاملComparative Study of the Effects of Combined Oral Contraceptives in Hemostatic Variables
Thrombotic risk is associated with the estrogen dose and type of progestin in combined oral contraceptives. Studies published since 1990 showed that third-generation progestins have larger risk to contribute to thrombosis development than the second-generation. However, there are conflicts in the literature regarding the thrombotic risk associated to the drospirenone progestin. So, this study a...
متن کاملCancer and contraception
As a result of advances in cancer diagnosis and treatment, young women within the reproductive-aged group are now more likely to survive cancer. Reproductive-aged women with cancer may be interested in deferring pregnancy either temporarily or permanently at cancer diagnosis, during therapy or after treatment. Currently, there are limited guidelines to aide clinicians in managing the contracept...
متن کاملOral contraceptives and the risk of gallbladder disease: a comparative safety study.
BACKGROUND Recent concerns have been raised about the risk of gallbladder disease associated with the use of drospirenone, a fourth-generation progestin used in oral contraceptives. We conducted a study to determine the magnitude of this risk compared with other formulations of oral contraceptives. METHODS We conducted a retrospective cohort study using the IMS LifeLink Health Plan Claims Dat...
